Any unused services remaining after the 12-month membership period are non-refundable. Services not included in the wellness plan are due at the time they are provided. Wellness plans automatically renew annually unless canceled. Plans may be canceled at any time; however, cancellation before completion of the initial 12 monthly payments may result in additional charges to account for discounts received. The $39.95 enrollment fee is non-refundable. Monthly payment plans require:

  • A valid debit/credit card or a voided check for automatic payments
  • A valid photo ID
  • Payment of the enrollment fee and the first month’s installment at the time of enrollment
